We begin with a detailed assessment of your medical history, current health status, movement capacity and personal goals. This ensures your program is safe, appropriate and aligned with your needs.

Your program is designed specifically for you, taking into account your condition, symptoms and daily demands. Exercises are modified and progressed carefully to support long-term health and sustainability.

3. In-Home & Community-Based Sessions
Sessions are delivered one-on-one in your home or a suitable community environment, making exercise accessible, comfortable and relevant to your daily life.

4. Ongoing Support & Adaptation
We closely monitor your response to exercise, adjust your program as needed and provide education to help you manage your condition confidently and safely.

Do you have a chronic condition?
You may be eligible for a GP Chronic Condition Management Plan which entitles you to up to 5 Medicare-subsidised allied health sessions annually.
Ensure your GP refers you for Exercise Physiology sessions to utilise them with Stay Active.
